The notification itself is a form, it has an approved form (Order of the Ministry of Internal Affairs of Russia dated December 10, 2020 No. 856) and is filled out by the applicant, that is, the one who provides the foreigner with his own housing.
The form has two sides - front and back . The front (1 page) contains information of the foreign citizen, and the back of the applicant (otherwise the receiving party).
The notification form is divided by a line, the lower part of which is called a “tear-off coupon”. After registration, a tear-off coupon is handed over to you.

Fines
Article 18.9 of the Code of Administrative Offenses of the Russian Federation is devoted to the liability provided for violation of migration legislation. It contains a list of violations related to the stay of foreign citizens and stateless persons in Russia, and includes fines in the amounts:
- from 2000 to 4000 rubles - for individuals;
- from 40,000 to 50,000 rubles - for officials;
- from 400,000 to 500,000 rubles - for legal entities.
Thus, ignoring the requirements of current legislation in the field of migration entails significant penalties.

What changes have occurred in 2021?
Note! Migration registration forms for 2021 are not valid at the moment! The information in this section is out of date and is provided for informational purposes only. New forms can be downloaded here.
Until March 20, 2019, the sample form established by law was used. A new template, rules, and filling requirements have now been adopted. The form was first changed in 2021, and significantly expanded in 2020.
Thus, according to the Government Resolution (On Amendments to the Rules for Registering Migrants), it has now become mandatory to indicate the details of legal documents that must confirm ownership of the property at which the foreign citizen will be registered.

Changes that have occurred are highlighted in red.
Among the main changes:
- Section No. 1 – when entering personal information about a foreigner who is being registered, his telephone number is indicated. This information had to be submitted before, but now this column has moved to the front of the form;
- it is now necessary to provide information about official representatives;
- a new clause has appeared - the mandatory indication of the address at which the migrant was previously registered. If a foreigner came to Russia for the first time, this should be indicated;
- section No. 2 – here the “house” field was previously duplicated, now this defect has been eliminated and the column with the building number has been reduced to two characters;
- Many people find it difficult to add the TIN column where they need to indicate the full name of the organization. This applies to those citizens who repeatedly come to Russia, have issued a labor patent and are registered with the tax authorities;
- if a citizen comes on a private visit, there is no need to indicate the organization;
- Previously, the tear-off coupon had a field where the date of departure of a foreign citizen from the country was entered, but since forms have now been introduced notifying the departure of a foreigner from the territory of the Russian Federation, this column has been removed for filling out.
According to the new rules, it is allowed to use abbreviations when filling out the migration registration form, when indicating the names of organizations, and abbreviations, if justified, do not distort the meaning.
Changes to the registration form came into force on March 24, 2020 (Order No. 180 of the Ministry of Justice). At the same time, the registration deadlines have not been changed; the migrant is also required to register no later than seven days after arrival in the country; violation of this rule may result in a fine and other sanctions.

The front side, which is the main part, contains information regarding:
- Full name, citizenship, date, place of birth, gender;
- information about an identity document;
- information about proof of the right to stay in the Russian Federation (if any);
- dates, purpose of entry, profession, duration of stay in the country, information about the migration card;
- information about legal representatives (in terms of children and incapacitated persons);
- information about the address of the previous place of stay in the country.

How to fill out a notification correctly and avoid mistakes?
It is important to be careful and fill out the form correctly, otherwise, if you make a mistake, you will have to submit it again. We understand perfectly well that there are huge queues in these institutions. Therefore, following the prompts, we will fill out the form according to all the rules.
The form must be filled out legibly in Russian, in capital block letters, using a pen with black or dark blue ink, without abbreviations or acronyms. You can fill it out manually; to do this, you need to download the notification to your computer and print it out. You can also fill out the downloaded form by typing on your computer. Or you can contact a service that will prepare all the necessary documents automatically for subsequent registration, otherwise the 7docs system. Next, using a sample example, we will issue a notification according to the rules.
The front side (page 1) is filled out according to all the rules and in the field “1. Information about the person subject to registration at the place of residence" we enter the information:
- Full name and surname (we copy from the migration card or visa);
- Citizenship or nationality;
- His date of birth, and then on the same line we indicate his gender (o);
- In what state and city was you born?
- Document type;
- What date was it issued and the validity date of the document;
- If you have a visa or residence permit - that’s all, then we indicate the series and number of the document, date of issue and validity period;
- Purposes of the trip (about);
- You can indicate your profession (see the employment contract);
- Dates of entry and exit (see migration card or visa);
- Series and number of migration card;
- If the notification is filled out for a minor child, we indicate information about legal representatives (full name);
- If the notification is issued to renew registration, please indicate your previous place of residence (address).
After the separation line, we fill out information confirming that the foreign citizen will be registered at the place of stay and indicate the address where he plans to live in the Russian Federation. Next, we note the date of stay.
The reverse side (page 2) contains information about the receiving party.
It is filled in according to all the rules and in the field “2 Information about the place of stay” we enter information about the place of stay (address, phone number of the receiving party).
Next, in the “3 Information about the receiving party” field, be sure to indicate who the addressee of the notification is - an individual or legal entity. An individual fills in only the passport details and registration address.
For the legal side, fill out the following fields:
- Full name of the person
- Date of Birth
- Document type;
- What date was it issued and the validity date of the document;
- Legal address, then indicate the telephone number along the line;
- Name of company;
- Actual address;
- TIN.
Below, after the separation line, we enter the full name of the receiving party and put a signature in two columns. Next, we print it out so that the form consists of one page (front side and back).
To submit documents to the Department of Migration (Migration Affairs), we must make two copies of the notification, copies of all pages of the foreign passport, a copy of the migration card and visa (if any).
Additionally, you will need a copy of the certificate of ownership of an individual, copies of constituent documents and a copy of an employment contract with a confirmed mark for a legal entity.
It is important not to delay submitting documents; it is better to complete the registration procedure on time.
More details about the dates in 2021:
- A citizen of Ukraine must register within 7 days. Can legally stay on the territory of the Russian Federation for 90 days.
- Citizens of Uzbekistan, the rules state, must register within seven days. You can legally stay in Russia for 90 days.
- Citizens of Tajikistan will be able to submit a notification within 15 days, and stay in our country for as long as 90 days.
- For citizens of Armenia, registration is carried out according to the rules that apply to the EAEU countries, that is, documents can be submitted within a month from the date of arrival. You can legally stay in Russia for 90 days. And if there is an employment contract, then you can register for 1 year.
Each of these citizens has the right to renew registration or arrive again after 3 months.
Why do we only have current data?
When searching for forms on the Internet, you should pay attention to their relevance. The fact is that forms are constantly changing. These changes may be minor, but despite this, it is prohibited to use outdated forms - they simply will not be accepted for consideration.
