Program conditions in 2021
The conditions for participation in the program are established by clause 1 and clause 7 of Appendix 8 to the State Program of the Russian Federation “On the Development...”, approved. Decree of the Government of the Russian Federation “On approval...” dated December 26, 2017 No. 1640. To receive allowances under the “Zemsky Doctor” program, a specialist must:
- have a medical education;
- move to work in a populated area with a population of up to 50 thousand people;
- conclude an employment contract with a medical institution for a period of at least 5 years.
Previously, doctors and paramedics who were under 50 years of age could become participants in the program. On November 2, 2021, the Decree of the Government of the Russian Federation “On the Introduction...” of October 18, 2019 No. 1347 came into force, which abolished all age restrictions - now all doctors who meet the criteria listed above can receive state support.
The minimum payment amount, according to clause 5 of Appendix 8, is:
- for a paramedic – 500 thousand rubles;
- for a doctor – 1,000 thousand rubles.
If the settlement is located in a remote or hard-to-reach territory (the list of such territories is approved by regional authorities), the amount of payment increases:
- for a paramedic – 750 thousand rubles;
- for a doctor – 1,500 thousand rubles.
Doctors who move to the Far East, Far North or the Arctic zone of the Russian Federation will receive the maximum allowance:
- paramedics – 1,000 thousand rubles;
- doctors – 2,000 thousand rubles.
Nuances of implementing a program for rural doctors in Russia
Potential rural medical workers are often faced with issues that are not addressed in the official explanations of the Compulsory Medical Insurance Fund.
So, people are wondering, what should families of doctors do? How much allowance will they be paid? What should they do correctly so as not to lose their opportunities?
Let us repeat, a rural doctor is involved in the project described. That is, it was written for a doctor. Nothing is indicated in the documents about family connections. Each doctor is entitled to a million .
If two doctors got married and decided to move to the outback, then each of them must do the following:
- Collect the package of the above documents.
- Apply to the department of the Ministry of Health.
Young female doctors who have received raises worry that their money will be taken away from them if they decide to have children. In this case, the employment relationship is not interrupted. While a woman is on maternity leave and maternity leave, they are waiting for her in the hospital (they save her place). Therefore, no one will take her money away.
Taxation of payments
In the period from 01/01/2018 to 01/01/2020, payments assigned to doctors under the “Zemsky Doctor” program were subject to income tax. The reason for this situation was that from the beginning of 2021, the source of funding for the program was not the compulsory medical insurance fund, as was previously the case, but the federal budget. After this, payments began to fall under clause 37.2 of Art. 217 of the Tax Code of the Russian Federation, therefore, taxes began to be withheld from them. As a result, the specialist received not 1,000 thousand lifting rubles, but only 870 thousand rubles.
In 2021, the Federal Law “On the Introduction...” of June 17, 2019 No. 147-FZ, paragraphs. "d" clause 1 art. 1 of which exempted from taxation income received by doctors in the form of one-time compensation payments. Those who received EDV in the period from 01/01/2018 to 01/01/2020 can return the overpaid tax. To do this, you need to write an application to the regional Department of Health with the details of the account to which you want to transfer the money.
Please note that only 1,000 thousand rubles are not subject to personal income tax, so you will still have to pay tax on a larger amount.
Results and prospects of the program
The Russian government regularly sums up the interim results of the impact of lifts on the medical care of rural residents. The conclusions drawn are mostly positive.
According to statistics, more than 20 thousand specialists have already moved to the village, using the lifts to purchase housing. Some federal subjects have fully provided rural citizens with qualified doctors.
Taking into account the positive results of the program, it was decided to extend its validity until 2021.
Results of the program in Crimea
In the regions of Crimea, more than 50 healthcare workers worked under the “Zemsky Doctor” program in 2021.
They received compensation payments in the amount of 1 million rubles. 26 specialists started working in medical organizations.
The program was financed in 2021 from the territorial branch of the Compulsory Medical Insurance Fund and from the republican budget in the ratio of 60 and 40 percent, respectively.
What documents are needed
To receive a payment under the state program, you need to submit the following documents to the regional Department of Health:
- application for registration as a participant in the program (the form and sample for filling it out will be provided to you by the Department);
- passport;
- diploma of education in the field of health care;
- a copy of the work book;
- SNILS and TIN;
- a copy of the employment contract concluded with the medical institution;
- The order of acceptance to work;
- Bank account details for transferring funds.
Program description
At the end of 2011, the law “On Amendments to the Federal Law “On Compulsory Health Insurance in the Russian Federation” was adopted, which detailed some aspects of the stages of implementation of the program aimed at stimulating the influx of medical workers with higher education to work in the Russian village.
Support under this program provides for the allocation of a million to each young doctor who decides to abandon the benefits of the city in favor of moving to the countryside. Relocated paramedics can count on 500 thousand.

Housing under the Zemsky Doctor program
Housing under the Zemsky Doctor program can be purchased using the compensation payment issued upon relocation. It is important to remember that the money has a specific purpose - it cannot be spent on current needs or used to cover loans received before moving. You can buy a house, apartment or plot for construction. In many settlements, even 1,000 thousand rubles is enough to buy decent housing, or pay off most of it when applying for a mortgage. The legislator does not establish special requirements for housing, so a young specialist can decide for himself where he wants to live.

Answers to basic questions
Having found out until what year the “Zemsky Doctor” program is valid, it is important for graduates of medical universities to get answers to other questions.
- If the arriving young family is both doctors with higher education. Will they give each one a million? Yes, both doctors are entitled to 1 million rubles.
- The compensation payment is paid from the funds of the Compulsory Medical Insurance Fund, which sums up regional and federal revenues.
- Considering the stimulating nature of the funds and their intended purpose, doctors do not need to pay personal income tax on the amount received.
- If the doctor decides to return to the city before the end of the 5-year contract, part of the money for the unworked period will have to be returned.
- If work is done in bad faith, the contract may be terminated. Money is refundable in full.
- Is maternity leave included in the Zemsky Doctor program? No, it is not taken into account in the five-year contract.
Be sure to carefully study the agreement with the Ministry of Health, as it contains clauses on fines and penalties. How to become a participant in the Zemsky Doctor program
The procedure for implementing the program is determined independently by the authorities of each region - to find out how to receive a payment in a particular city, you need to familiarize yourself with local legislation.
In general, in order to become a participant in the program, you must:
- fill out an application for payment;
- prepare documents attached to the application (their list is given above);
- submit documents to the regional office of the Ministry of Health;
- wait for the decision to assign payment (refusal to assign payment).
The terms for consideration of an application at the federal level are not regulated; they will also have to be looked for in regional legislation. As a rule, a decision on a submitted application is made within 30 days from the date of its receipt. Within 3 days from the date of the decision, the applicant receives a written notification (in case of refusal, in addition to basic information, it also indicates the reasons why payment will not be assigned).
List of documents
The package of papers may vary depending on the region and individual cases. General list of documents:
- a copy of the employment contract with a medical organization, the duration of which is at least 5 years;
- passport;
- diploma with an insert about the specialization received;
Diploma sample - a certificate indicating the place of residence in the locality;
- a copy of the employment order;
- SNILS;
- bank account number;
- an application requesting to be placed on a waiting list to receive financial support.
Important! When visiting the MHIF, a citizen will need to sign a consent to enter into a contractual relationship with the Ministry of Health.
When contacting an organization, you should provide not only originals, but copies of papers. If it is not possible to visit the MHIF in person, a medical worker has the right to fill out an application remotely.

To do this, the doctor should prepare all the papers and send them by mail. It is also possible to submit an application through the official MHIF website by filling out the form and attaching copies of documents.
Important! When submitting an application remotely, all documents must be notarized.
Employees must review the application sent to the Compulsory Medical Insurance Fund within 1 month. If a positive decision has been made, the doctor will be invited to the Department to formalize an agreement for payment of compensation.
Ogonyok asked young teachers about their work
The statistics of the Ministry of Education for 2021 provide figures close to Rosstat data: 1,034,626 teachers and 22,142 additional education teachers worked in schools. Well, the statistics do not show the shortage of personnel in recent years - not 12 thousand, not 150 thousand, much less 180 thousand teachers! The website zemteacher.edu.ru (launched under the state program) currently lists 3,565 open (what they are - just below) vacancies in rural schools. According to the Ministry of Education, 7,200 people are already applying for these places. This means that teachers will face a competitive selection process. In addition, every year 33 thousand young specialists come to the school (this is approximately half of the graduates of pedagogical universities), as stated by acting. O. Minister of Education Olga Vasilyeva. So what kind of deficit are we talking about?
Maybe the staffing situation is only good in the cities, while the periphery is groaning from a shortage of teachers? It turns out that this is not true either. When this material was being prepared, at the request of Ogonyok, information came from the Ministry of Education of the Russian Federation: currently in Russia there are 26 thousand rural schools, in which 420 thousand teachers work. If we take into account that 3,789 thousand children study in these schools (data from the collection “Education in Numbers”, 2021), then the picture will be simply fabulous: 9 students per teacher. If the conditions in rural schools were also good, and teachers’ salaries were higher, what more could one wish for?
What benefits are provided?
Following the current legislation, you can rightfully count on financial compensation of one million for doctors in rural areas who decide to carry out permanent work activities in this area.
To receive this amount, you must complete the application documents at the local government office.
State support is provided only for qualified doctors with higher education, working in a specialty obtained at the university.
The minimum work experience is at least five years. In a situation where the doctor decides to terminate the contract before the appointed time, he will need to return a share of the financial resources.
In addition to money, government authorities provide a list of benefits:

A mandatory nuance when allocating 1,000,000 Russian rubles to young professionals is to spend the money only to improve living conditions.
How to count deficit
It turns out that no one can say how many teachers are missing. And most importantly - which ones. Basically, all lessons in schools are taught in all subjects. Yes, some teachers complain about the heavy workload, but few of them will want to voluntarily give up their hours - this will hit their salary.
As experts say, today there is no exact definition of what to call a staff shortage in a school. Some people think this way: there are children, there are classes, but there is no subject specialist, which means there is a shortage. Others suggest counting by the number of teaching hours at school, dividing them into rates of 18 hours per week.
But who at school would agree to work at one rate (except for Moscow and St. Petersburg)? Especially in a village, where for 18 hours a teacher will receive from 12 to 14 thousand rubles with all allowances and bonuses.
A completely different picture of the shortage of teachers will be obtained if we take into account that in a single- or small-class school (and these are the majority in villages), a teacher is not prohibited from combining teaching of different subjects. If the school is large, the teacher can work two jobs, or even more. If someone's place becomes vacant, then the remaining teachers share their workload among themselves. And this is normal practice.
Benefits and payments
According to Federal Law No. 326, doctors who moved to a village for the purpose of working in their profession are entitled to receive compensation payments.
Their registration is possible not only if all criteria are met, but also if you have a work experience of 5 years. In addition to money, doctors are entitled to the following benefits:
- provision of a home for living;
- allocation of land for further construction of a house on it;
- reimbursement of a certain amount of the loan taken to purchase a home.
Depending on the region, young professionals are often given discounts on utility bills and are paid at an increased rate. The exact list of benefits can be found in local authorities.
If a female doctor participates in the program and she decides to go on maternity leave, she is not obligated to return the compensation received. The employment relationship with her is not canceled and her position and place in the medical institution are retained.
Important! The amount allocated to a young specialist as a participant in the “Zemsky Doctor” project is not subject to personal income tax, since the money has a target value and serves as an incentive to work in rural areas.
How much compensation can a young doctor claim?
The exact amount of funds to be paid under the program is determined after the approval of the state budget. The allocated money is distributed among the regions where the final amount of payments is formed.
Important! Funds are allocated for each citizen individually.
The maximum amount of compensation that a young doctor who decides to take part in the project can claim is 1 million rubles. Every year in Russia, officials consider the possibility of increasing the volume of assistance to young doctors working in villages. In 2013, 2.8 billion rubles were allocated for the program, in 2015 the amount was 3.2 billion rubles, and in 2016 4 billion rubles.
In 2020–2021, it is planned not only to increase the amount of funds to help doctors, but also to consider the possibility of abolishing age restrictions for citizens. Projects aimed at supporting medical workers include not only the Zemstvo Doctor program, but also the Zemstvo Paramedic program. The amount allocated to paramedical personnel is 500,000 rubles.
Where to go for money
A young specialist who has taken part in the “Zemsky Doctor” program must apply to the Compulsory Medical Insurance Fund in order to receive payments. Employees must pay compensation within 30 days after a positive decision is made upon consideration of the application. The amount that the state provides to a citizen under the program must be transferred to the municipal budget within 3 days. Once funds are received, local governments must add the missing portion of the money and transfer the compensation to the participant's account.
